A clear pathway before you travel
Rectus diastasis is more than a change in abdominal appearance. In selected patients it may be associated with abdominal bulging, weakness of the abdominal wall and umbilical or epigastric hernias. The correct treatment begins with a specialist surgical assessment and a procedure chosen around the anatomy, tissue quality and individual goals of the patient.
International patients may begin with a video consultation. When the first assessment is performed online, an abdominal wall ultrasound or, preferably, a CT scan must be reviewed in advance. Routine imaging is not required before an in-person consultation unless specifically requested after the clinical examination.
- Direct consultation with Dr. Salvatore Cuccomarino.
- Personalised indication for REPA, MILA or an alternative procedure.
- Preoperative planning, hospital admission and follow-up coordinated in Milan.
Minimally invasive abdominal wall surgery
REPA and MILA: two different surgical goals
The choice between REPA and MILA is made only after a complete assessment. The two procedures share a minimally invasive approach, but they address different clinical and aesthetic needs.
REPA
REPA is an endoscopic pre-aponeurotic repair designed to reconstruct the linea alba, correct rectus diastasis and treat suitable associated midline hernias through three small suprapubic incisions. The repair is reinforced with an ultralight mesh to improve stability and reduce the risk of recurrence.
It is primarily functional abdominal wall surgery and is generally considered for patients without a major excess of skin. REPA always requires one night of hospitalisation.
MILA
MILA—Minimally Invasive Lipo-Abdominoplasty—combines REPA with VASER liposculpture and Quantum RF skin tightening. It is intended for carefully selected patients who require both reconstruction of the abdominal wall and a more complete three-dimensional remodelling of the abdomen.
MILA aims to improve contour, adipose distribution and skin retraction while avoiding the long transverse scar of a traditional abdominoplasty. It is not suitable for every patient and requires an individual clinical evaluation.

Experience and surgical specialisation
Dr. Salvatore Cuccomarino, MD, FACS
Dr. Salvatore Cuccomarino is a general surgeon with more than 30 years of experience and a specific focus on minimally invasive abdominal wall surgery. In 2017 he performed the first REPA procedure in Europe and has since developed one of the largest international experiences with this technique.
His work also includes the development of MILA, an integrated minimally invasive approach combining abdominal wall repair, VASER-assisted liposculpture and Quantum RF skin tightening. Every indication is based on a direct clinical assessment and realistic discussion of the expected functional and aesthetic outcome.
