MILA Surgery in Milan: Minimally Invasive Lipo-Abdominoplasty

Advanced minimally invasive abdominal contouring in Milan

MILA surgery in Milan: minimally invasive lipo-abdominoplasty

MILA combines minimally invasive rectus diastasis repair with VASER liposculpture and Quantum RF skin tightening. It is designed for carefully selected patients who need both abdominal wall reconstruction and three-dimensional contouring without the long scar of a traditional abdominoplasty.

REPA
Repair of rectus diastasis and selected midline hernias
VASER
Selective liposculpture of the abdomen and flanks
Quantum RF
Internal radiofrequency for tissue contraction and skin tightening
Minimal scars
Small access points rather than a long lower-abdominal incision

Function and shape

What is MILA?

MILA stands for Minimally Invasive Lipo-Abdominoplasty. It is not simply liposuction, a mini-abdominoplasty or a cosmetic skin treatment.

The procedure addresses three anatomical layers that often change together after pregnancy, weight variation or progressive loss of abdominal tone: the abdominal wall, the distribution of subcutaneous fat and the ability of the skin and superficial tissues to retract.

The wall is reconstructed through the REPA component. VASER is used to reshape the adipose layer with greater precision, while Quantum RF is used internally to promote contraction and compacting of the superficial tissues.

The central concept: MILA does not start from the idea of removing skin or simply aspirating fat. It begins with the diagnosis of what is causing the abdominal prominence and then combines the necessary components in a tailored surgical plan.

One integrated surgical strategy

The three components of MILA

Each component has a different role. The final plan is adapted to the patient rather than applied as a fixed package.

01 · Reconstruct

Rectus diastasis repair

The REPA component reconstructs the linea alba and can treat selected umbilical or epigastric hernias through small suprapubic access points.

02 · Reshape

VASER liposculpture

Ultrasound-assisted liposculpture refines the abdomen and flanks while respecting the natural anatomical lines and transitions.

03 · Tighten

Quantum RF

Internal radiofrequency is used to improve tissue contraction, compactness and skin adaptation over the reconstructed abdominal wall.

Careful patient selection

Who may be suitable for MILA?

MILA is most useful when rectus diastasis is associated with a need for contouring and tissue contraction, but the amount of loose skin does not require a traditional abdominoplasty.

MILA may be considered when there is:

  • Rectus diastasis after one or more pregnancies.
  • A persistently prominent or poorly contained abdomen despite stable weight and exercise.
  • Localized adiposity of the abdomen and flanks requiring precise contouring.
  • Mild to moderate skin laxity with sufficient residual capacity for retraction.
  • A wish to avoid, when clinically appropriate, the long scar of a traditional abdominoplasty.
  • Realistic expectations and willingness to follow the postoperative programme.

MILA is not suitable for everyone. Major skin excess, a severe abdominal apron, extensively damaged skin or expectations that cannot be met with a minimally invasive approach may make traditional abdominoplasty or another strategy more appropriate.

Beyond simple liposuction

VASER and Quantum RF work on different tissue layers

VASER uses ultrasound energy to facilitate controlled treatment of the adipose layer. Its purpose within MILA is not general weight loss, but selective remodelling of the abdomen and flanks in harmony with the reconstructed abdominal wall.

Quantum RF is an internal radiofrequency technology used through dedicated fine cannulas. It acts below the skin to support contraction and tissue compactness. This is different from superficial cosmetic radiofrequency treatments performed without surgery.

The technologies do not replace surgical judgement. Their value depends on correct diagnosis, safe use and a realistic assessment of the skin’s capacity to retract.

The images below illustrate the type of combined functional and aesthetic change sought with MILA in a selected patient.

Choosing the right operation

MILA, REPA or traditional abdominoplasty?

There is no universally superior procedure. The correct choice depends on the abdominal wall defect, skin quality, fat distribution, previous scars, associated hernias and the patient’s priorities.

REPA

Wall repair first

Best suited when the main objective is minimally invasive correction of rectus diastasis and selected midline hernias without advanced body contouring.

Abdominoplasty

Major skin removal

May be necessary when skin excess is extensive or cannot reasonably retract, accepting a longer scar and a more demanding postoperative course.

International patient pathway

From video consultation to surgery in Milan

The pathway can begin remotely, but the final surgical plan requires a complete specialist assessment. Photographs alone are never sufficient to confirm suitability or provide a definitive surgical plan.

Step 1

Initial contact

Send a brief medical history, previous abdominal operations, current symptoms and your treatment goals.

Step 2

Video consultation

For a Google Meet consultation, an abdominal wall ultrasound or preferably a CT scan must be reviewed in advance. Supporting photographs may also be requested.

Step 3

In-person assessment

A direct examination confirms whether MILA, REPA, traditional abdominoplasty or another strategy is the safest and most appropriate option.

Step 4

Surgery and follow-up

Surgery is planned in Milan with an overnight hospital stay and postoperative checks before the patient is cleared to travel home.

Specialist experience

A European reference in minimally invasive diastasis surgery

Dr. Salvatore Cuccomarino has more than thirty years of experience in minimally invasive surgery and a major clinical practice dedicated to rectus diastasis, abdominal wall reconstruction and advanced abdominal contouring.

His team performed the first European MILA procedures and has developed extensive experience in integrating REPA, VASER and internal radiofrequency within a single tailored surgical strategy.

The indication is always personalised. The objective is not to offer MILA to every patient, but to identify the operation that best balances function, contour, scars, safety and realistic expectations.

Frequently asked questions

Questions about MILA surgery

Is MILA the same as a mini-abdominoplasty?

No. MILA is a combined strategy that reconstructs the abdominal wall and integrates VASER liposculpture and Quantum RF tissue tightening. A mini-abdominoplasty mainly removes a limited amount of lower abdominal skin.

Does MILA leave a long scar?

MILA uses small access points positioned discreetly above the pubic area. It does not require the long transverse scar of a traditional abdominoplasty, although every surgical incision produces a scar.

Can MILA remove a large amount of loose skin?

No. MILA relies on the residual ability of the skin and superficial tissues to retract. Patients with major skin excess may require traditional abdominoplasty or a different procedure.

Is MILA a weight-loss procedure?

No. VASER is used for selective contouring, not for general weight reduction. Candidates should ideally be close to a stable and appropriate body weight.

Do I need a CT scan before an in-person consultation?

Not routinely. Imaging is specifically required in advance when the first assessment is performed by video consultation. During an in-person visit, the need for additional tests is decided after direct examination.

How long should I remain in Milan?

The recommended stay depends on the extent of treatment, your health, travel distance and postoperative progress. The exact plan is agreed during the consultation, and you should not book a return journey until the surgical team confirms the appropriate timing.
